Important Note: Insurance plan contracts can change frequently. To confirm active coverage and in-network status, we recommend verifying participation directly with your insurance provider using our Tax ID 68-0501385, or by contacting our business department at (866) 358-0202.

If you have an HMO insurance plan, please ensure you have obtained a referral from your HMO provider prior to your appointment, as this is required for your visit with Dr. Joseph.
